About
Scott Decker
Founder & Managing Principal, BluCascadia Advisors LLC
The Operating Background
Scott Decker brings 40 years of healthcare IT operating experience across the full spectrum — health systems, payer platforms, ambulatory care systems, home health technology, revenue cycle operations, and the clinical and operational bridges between them.
He's operated at three distinct scales: as a founder building healthcare IT startups from concept to $40M ARR; as a large-scale operator scaling enterprise healthcare IT platforms to $400M+ in revenue; and as a PE-backed executive navigating ownership cycles, enterprise customer relationships, and technology transformation. He's also been advisor and early-stage board member to healthcare IT founders navigating startup challenges.
This breadth across scale and perspective is deliberate. Understanding how health system incentives shape payer strategies, how payer incentives shape ambulatory workflows, how home health integrates with primary care networks, how RCM platforms connect clinical and financial data — and understanding how to build that at startup speed AND scale it at enterprise speed — that's where the insight is. It's the difference between being a specialist in one vertical and understanding how healthcare IT actually works across the entire system at every operating scale.
This is not advisory drawn from case studies. It's insight from three different operating chairs — the founder building the initial product and business model, the large-scale operator managing enterprise complexity, and the PE-backed executive navigating growth and transformation. All three perspectives inform every engagement.
Current Activity
Scott's advisory work spans the full diligence lifecycle for PE firms — from thesis development through market mapping, competitive assessment, management evaluation, and post-acquisition strategy.
He maintains active board governance roles at healthcare IT portfolio companies spanning revenue cycle operations, home health services, and health system technology — bringing direct large-scale operating and technology oversight experience to BluCascadia's advisory work.
He also advises emerging healthcare IT company founders navigating product-market fit, scaling, and the transition from startup to enterprise operations. This founder advisory complements his PE work: the same operating experience that informs due diligence also informs strategic guidance for scaling companies.
The result: BluCascadia's advisory is grounded in current operating reality across all three perspectives — through active board roles at scale, through ongoing founder relationships across the healthcare IT landscape, and through direct PE engagement work spanning thesis development through execution.
The Technical Foundation
Scott holds a Computer Science degree — an uncommon foundation for a healthcare IT executive, and a deliberate advantage. The ability to evaluate technology at depth, not just from a business perspective, informs every due diligence engagement and every technology advisory mandate.
When AI became the defining force in healthcare IT, Scott didn't delegate the analysis. He built the tools.
The Intelligence Engine
BluCascadia's proprietary IE2 system was built because the alternative — starting each engagement from scratch, relying on generalist analysts, or purchasing expensive and often stale market reports — was inadequate for the depth PE clients require.
IE2 maintains a persistent, real-time intelligence layer across the healthcare IT landscape: monitoring competitive signals, market movements, and sector dynamics across post-acute care, home health, RCM, and adjacent markets. Every BluCascadia engagement benefits from cumulative knowledge, not just individual expertise.
Current Focus
BluCascadia operates a deliberately limited engagement roster — three to five engagements per quarter — to ensure principal-level attention on every mandate. Current clients include PE firms with $500M–$10B AUM actively deploying in healthcare IT.
Scott maintains active board governance roles at healthcare IT portfolio companies and advises emerging healthcare IT company founders, ensuring BluCascadia's advisory work stays grounded in current operating experience — from the founder perspective through large-scale enterprise operations and PE-backed growth.
